Know Your Number: Cardiovascular Disease Risk Assessment
In collaboration with BioSignia, Inc., we will provide your employees with a complete cardiovascular risk assessment. This risk assessment includes:

  • A complete lipid panel (total cholesterol, HDL, LDL, triglycerides), blood glucose, and blood pressure. Using these results along with biometric measurements such as height, weight, and waist circumference, each participant will receive a computer generated graph, depicting 5-year risk of onset for heart disease, stroke, type 2 diabetes, and heart failure, at time of service (example included).
  • Unlimited access to personal health data, laboratory results and educational materials to each participant via the internet.
  • 6-month follow up assessment for high risk patients, tracking progress and evaluating the impact of lifestyle modifications and pharmacist recommendations.
  • Private consultation, delivered by clinical pharmacists specially trained in chronic disease state management.
